I am a clinical pharmacist at HAHC Hospital, I ensure safe, effective & rational use of medications, working closely with physicians, nurses & other healthcare professionals, I review prescriptions, monitor drug therapy, contribute to antimicrobial stewardship and ADR while promoting evidence-based medicine. I do patient counselling on proper medication usage, drug reconciliation, and ensure every pt. receives appropriate and effective pharmacotherapy.
